For our large animal clients: this form will contain the information needed for our office to issue you a Veterinary Feed Directive (prescription for medicated feed). The Veterinary Feed Directive was created by the FDA to monitor levels of antibiotics in feed as a pursuit to decrease antibiotic resistance in food animals.
